Your team should consist of 10 student players (5 MALES/5 FEMALES) and 1 Teacher to serve as the team manager.

Please make sure you have a complete team and know ALL member names BEFORE completing this form!

You WILL NOT be allowed to drop and add players once the season begins.

If the players on the list fail to show up for your game, your team will forfeit and will be removed from the Intramural League.
