Your team should consist of 8 student players (any gender).

Please make sure you know first and last name of ALL members BEFORE completing this form! 

 

You WILL NOT be allowed to drop and add players once the season begins. If the players on the list fail to show up for your game, you will play with less.

 

Feel free to sign up as an individual or group of less than 8.
